Please print this form, complete the top section, sign and date, then bring it to first practice/signup attended.

Note: Print instructions are for computers running Windows 11 and may not work for every computer.

1. To print, start by clicking on the form name at the bottom of this window.
2. Form appears in new window.
3. Click on the printer icon in the upper right corner.
4. Select PRINT in drop down window.
5. Window appears with form image and print options.
6. Select PRINT
